Submitting requests

When a user has added accessions to Shopping Cart, they should be able to submit a request for material.

Please add a button "Request material" that renders the Persona information form:

Note: please move "Internal request" to be the last form field.

Recaptcha check is required (see genesys-server#553 (closed)).

To upload designs, you'll need to enable LFS and have an admin enable hashed storage. More information
