Workflows: Define default action note

@c.ogwuike suggests that we add support for Default action note in the definition of a workflow Step. This default note is applied to the actions that are created in that step.

This ticket is to add "Default action note" to the step form. The field is optional.

API upgrades required:

• Add note to models
• Update workflow engine to copy the note from workflow step
